We are excited about the partnership you have elected to participate in this coming season as a mentor or mentee. You will find three forms to review:

1. The matching of mentors and mentees form

2. The evaluation form

3. Mentor guidelines

If you have not already done so, please contact your mentor/mentee this week. You can find who you have been matched up with on the attached form and then go to Zebraware under member profile for their phone numbers and email addresses. Please be sure to invite them to you varsity games.

Secondly, the evaluation form needs to be completed by all mentors on their mentee by the end of the season. The committee is requesting each mentee be evaluated 3 times during the course of the season. Please email the completed evaluation form to the email address below.
